KidsInclusive has been working with community partners for the past two years to develop a proposal for Coordinated Service planning as part of the Special Needs Strategy initiative. This multi-ministerial initiative was a response to long-standing concerns expressed by parents needing access to special needs services. Coordinated service planning is an intervention that will provide a seamless/integrated service delivery experience for the client – an intervention that will meet the required outcomes in an effective and efficient manner. Coordinated service planning is intended for children with multiple and/or complex special needs, requiring more than one community-based specialized service.
Coordinated service planning is comprised of three key components:
A single Coordinating Agency in each service delivery area through which families can access coordinated service planning for a range of services across sectors;
Dedicated Service Planning Coordinators, through each Coordinating Agency, who will lead coordinated service planning for families of children and youth with multiple and/or complex special needs by working with children’s services, health and education sectors; and
One coordinated service plan for each child/youth that takes into account all of his/her goals, strengths, needs, as well as all of the services that the child/youth is and will be receiving.
KidsInclusive is the proposed coordinating agency for KFLA and is currently waiting for formal approval to move ahead in this capacity. KidsInclusive will be partnering with Lanark Community Programs and Developmental Services of Leeds and Grenville to provide coordinated service planning in LLG. Again, formal approval is pending.
